REQUIREMENTS :
A) RE-ISSUE OF A PASSPORT
1. Application Form’s print out, filled online (https://embassy.passportindia.gov.in), to be submitted at the Embassy.
2. Photocopy of Passport, self-attested (Original passport to show)
3. Photocopy of Residence Permit and Work Permit, self-attested (Original to show)
4. Four recent colour Photographs with white background, Size: 5cm * 5cm.
5. A letter from the company, the applicant is working in. (Click here)
B) LOST/DAMAGED PASSPORT PASSPORTS
1. Application Form, filled online (https://embassy.passportindia.gov.in)
2. Photocopy of lost Passport, self-attested
3. Photocopy of Residence Permit and Work Permit, self-attested (Original to show)
4. FIR from Police, for lost passport along with English translation.
5. Four recent colour Photographs with white background, Size: 5cm * 5cm.
6. Self-explanatory note, explaining the circumstances of how the applicant lost his passport.
7. A letter from the company, the applicant is working in. (Click here)
C) CHILD'S PASSPORT
If applying for fresh passport, start with step 1, otherwise start with step 2.

Also, kindly note that with effect from 15 January 2023, the Registration of Birth Child service will be delivered through SEWA portal https://indianconsularservices.mea.gov.in/consularServices/ . a) Fill in Self sworn affidavit (Click here)
b) Fill in Self sworn Affidavit (NOC) (Click here)
c) Declaration of both parents that the minor child does not hold a passport of another country.
d) Proof of Indian citizenship of the parent(s)
e) Original Marriage Certificate (to show) and copy of the marriage certificate. (In case the Marriage Certificate is issued by Algerian Local Govt, certified true translation copy of Marriage Certificate duly attested/apostled by the Ministry of Foreign Affairs of Algeria is required alongwith copy of the original Marriage Certificate)
2. Application Form, filled online (https://embassy.passportindia.gov.in)
3. Photocopy of Passport, attested by parent
4. Photocopy of Residence Permit, attested by parent
5. Translated copy of birth certificate from the Ministry of Foreign Affairs along with photocopy of the original birth certificate.
6. Photocopies of parent's passport along with original and Residence Permit and Work Permit, self-attested
7. Four recent colour Photographs with white background, Size: 5cm * 5cm.
8. Dully filled in Annexure D (Click here)
D) Police Clearance Certificate
Issuance of a Police Clearance Certificate (PCC) by this Embassy is subject to receipt of fresh Police Verification Report from concerned authorities.

2. Photocopy of Passport, self-attested (Original passport to show)
3. Photocopy of Residence Permit and Work Permit, self-attested (Original to show)

Important: Applicants may please note that if applying for PCC with details other than those mentioned on their existing passport, they should first seek reissue of passport with changed/current particulars. It may also be noted that only after re-issue of passport with changed/current particulars, the PCC application against such reissued passport will be processed further for issue of PCC.
